New Patient Registration
Please note that we have recently been granted permission to change our boundary area and as a result this has reduced in size. The area remains the same from the surgery to The Avenue but now stops at Tebourba Way/ Commercial Road/Burgess Road, along the A35. If you live the other side of this boundary then we will no longer be taking registrations.
If you are within the practice area (East of A35, North of Millbrook Road and West of The Avenue, Southampton) then simply call into the surgery to complete the necessary forms. Once complete you will be offered a new patient check with the nurse. You will then be registered with the surgery and not one particular doctor. You can then see any of the doctors in the practice if they are available.
Medical treatment is available from the date of registration. Please contact reception for further information.
To save time you may want to print off a registration form (below), fill it out and bring it with you on your first visit to the practice along with the completed New Patient Questionnaire and/or opt out form which will save you time at the reception desk. Please note it will take up to 21 days for your registration to be finalised.
*We only register patients living within the black lines on the map below. Anyone currently registered here who then moves to within the yellow shaded area, can stay with the surgery but anyone outside this area needs to register elsewhere.
PLEASE NOTE - to register online you need to go to the 'registering online' tab to the right of this one and complete ALL documents - registration form, online medical questionnaire and data opt out form. We need these details available to us to get you on the system. If items are missing we will not be able to complete the process. If you are happy to share your medical information with other healthcare professionals and do not wish to opt out then drop us an email to email@example.com at the time of submitting the other information.
